Job Description 1:1 Personal Care Nurse (Must be a RN) Provide direct nursing care to a designated medically fragile student, which may include tasks such as tracheostomy care, gastrostomy tube feeding, seizure monitoring, and mobility assistance. Accompany the student throughout the school day, including on the bus to and from school, ensuring continuous care. Administer medications and perform treatments as prescribed by the student's healthcare provider. Develop and implement individualized health care plans (IHPs) in collaboration with the student's educational team. Maintain accurate health records and documentation of care provided. Communicate effectively with parents, school staff, and healthcare providers regarding the student's health status and needs.
