This application should be filled out for all applicants that desire to volunteer in the school setting - Lunch Buddy, Mentor, Classroom, Athletics.
ONLY COMPLETE THE ATTACHED MENTOR FORMS (Mentoring Form and Parent Mentoring Agreement Form) IF YOU DESIRE TO SERVE AS A BONA FIDE MENTOR. The completed forms should be submitted to the Parent Liaison at the school your child attends.
Please allow at least 7 business days for processing.
